（二）晚期（淋巴阻塞性病變期） 系淋巴系統(tǒng)增生、阻塞 所引起的表現(xiàn)，炎癥仍反復出現(xiàn)，故多數(shù)病例炎性和阻塞性病變 交叉重疊出現(xiàn)。1、淋巴結(jié)腫大和淋巴管曲張 反復發(fā)作的淋巴結(jié) 炎和淋巴結(jié)內(nèi)淋巴竇的曲張為導致淋巴結(jié)腫大的因素。腫大的淋 巴結(jié)和其周圍向心性淋巴管曲張形成腫塊，觸診似海綿狀包，其 中有硬核感覺，穿可得淋巴液，有時可找見微絲蚴。淋巴結(jié)腫大 多見于腹股溝。淋巴管曲張常見于腹股溝、精索、及大腿內(nèi)側(cè)。 今索淋巴管曲張常相互粘連而成索狀，不易與精索經(jīng)曲張相鑒別 ，且二者多并存。2、鞘膜積液 多見，輕者常為無癥狀，積液較 多者體積增大，皺褶消失，有下墜感而無疼痛，透光試驗陽性。 積液可為草黃色淋巴液或乳白色的乳糜液，積液沉淀中可找到微 絲蚴。3、乳糜尿 主要臨床表現(xiàn)之一。淋巴管阻塞造成腸干淋巴 管內(nèi)淋巴液返流，進入泌尿道內(nèi)形成乳糜尿，淋巴瘺處伴出血混 料類稱為乳糜血尿。常驟然出現(xiàn)，發(fā)作前可無癥狀，亦可有畏寒 、發(fā)熱、腰部、盆腔及腹股溝等處疼痛，繼之出現(xiàn)乳糜尿。一般 發(fā)病后持續(xù)數(shù)日或數(shù)周而自行停止，但勞累或進食油膩后可再誘 發(fā)。尿呈乳白色若混有血液則呈粉紅色。
